Scope of Position: The Bridge is seeking compassionate and motivated Scattered Site Case Managers to support our mission of transforming lives through comprehensive, evidence-based rehabilitative services. Scattered Site Case Managers provide community-based support to formerly homeless adults with serious mental illness and/or substance use disorders, delivering services primarily in clients' apartments and throughout the community. Responsibilities include conducting assessments, developing individualized Support Plans, linking clients to medical, mental health, and social services, and helping them build the skills and supports needed to maintain stable housing. Using a person-centered and strengths-based approach, Case Managers also facilitate groups, complete timely documentation, coordinate care with providers, and respond to crises as needed. This role requires empathy, cultural competence, and a commitment to empowering clients toward greater independence and stability.

Essential Position Functions:

  • These services are primarily provided in the client's apartment and in community settings.
  • Provide case management and interventions to high-risk clients, including conducting assessments and facilitating referrals.
  • Assist in identifying strengths, needs, abilities, and goals, and create related Support Plans in the electronic client records system.
  • Utilize Motivational Interviewing techniques to assist clients in developing the skills necessary to maintain their housing. Assist and encourage individuals to develop natural supports and include these supports in Support Plans and client-related meetings.
  • Timely completion of required documentation for every client contact as per agency policy. Ensure documentation meets regulatory guidelines for the level of care.
  • Co-facilitate groups if requested in areas including but not limited to wellness, leadership, housing, stability, employment and education, relationships, and healthy decision-making.
  • Provide case management services, including assistance with housing applications, referrals to both medical and mental health care, maintaining active benefits, escorting, compliance with service providers, and coordinating care with outside providers.
  • Attend case conferences with the High Risk Coordinator to formulate interventions.
  • Attend staff meetings, supervision, and trainings as required.
  • Assist in preparing the program for audits by ensuring compliance with requirements, including managing admissions and discharges, maintaining accurate census records, overseeing documentation, and handling incident reporting.
  • Collaborate with other departments, outside agencies, and treatment providers.
  • Ability to work with consumers/residents, families, community tenants, and staff in a caring and respectful manner, and with due understanding of and consideration for cultural differences.
  • Conduct apartment inspections on multiple floors, utilizing stairs. Provide escorts to clients throughout the community utilizing public transportation.
  • Immediately reports serious incidents, serious incident allegations, incidents, or sensitive situations to supervisors.
  • Complete incident reports in accordance with Bridge policy.
  • Account for consumers, perform diligent searches, and file missing person reports on consumers not accounted for in accordance with policy and procedure. Provide crisis intervention, as necessary, utilizing de-escalation techniques.
  • Any other duties as may be assigned, including the potential of supporting other programs within the agency portfolio.
Qualifications
  • Bachelor's Degree required.
  • Experience working with mental health, crisis intervention, and managing trauma-triggered behavior.
  • Excellent time management and organizational skills.
  • Team player who is stress resistant, flexible, creative, and open to feedback.
  • Willingness to perform CPR/First Aid/AED/ Naloxone.
  • Preferred knowledge of AWARDS.
  • Bilingual in Spanish is a plus.
  • Veterans or direct experience working with Veterans is preferred.
