Role Overview:

We are seeking a compassionate and dedicated Hospice Night Nurse. As a Hospice RN you will provide essential care and support to patients where ever they call home. Answer and respond to any calls over night for patients in need. This is a rewarding opportunity to make a meaningful difference in the lives of individuals during their end-of-life journey.

Your role:

  • Hospice heart: compassionate, kind and empathetic to your patients and their families
  • Nurse/Patient Relationship: Develops and maintains a therapeutic relationship through the health care continuum
  • Provide direct patient care services to hospice patients
  • Empower patient's caregivers by education them on care for their loved one
  • Provide coordination of care, ensure continuous assessment of each patient's and family's needs
  • Responsible for documentation and communication of care, medication, and observations made during care of the patient in order to track responsiveness and health of patient.
  • Responsible for providing appropriate patient/family education to help patient/family achieve a pain-free and comfortable status.
  • Provide on call assistance to patients in need
  • Admit patients to hospice services
