Responsibilities:
· Provide comprehensive primary and preventive care to patients of all ages.
· Conduct physical examinations, order and interpret diagnostic tests, and develop treatment plans.
· Diagnose and manage acute and chronic illnesses.
· Educate patients on health promotion and disease prevention.
· Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care.
· Maintain accurate and up-to-date patient records in compliance with state and federal guidelines.
· Prescribe medications and therapies as appropriate.
· Participate in community outreach programs to promote health awareness
- Engage in telehealth consultations to expand access to quality care while maintaining high standards of patient safety and confidentiality.
Responsibilities:
· Current and valid New York State Nurse Practitioner (NP) certification in Family Health.
· Licensed and registered as a Registered Professional Nurse (RN) in New York State.
· Graduate of a nurse practitioner education program acceptable to the New York State Education
· Department (NYSED) or certified as a nurse practitioner by a national certifying organization acceptable to NYSED.
· Completion of pharmacology coursework acceptable to NYSED.
· Minimum of 2 years of experience as an NP in a primary care or family practice setting preferred.
· Bilingual skills in Bengali or Spanish are highly preferred.
· Proficiency in using electronic medical records (EMR) systems.
