Care Navigator, Akron Older Adults

JFSA of AKRON is actively seeking a part-time (20 hours a week), Care Navigator to work with older adults to development individualized plans to achieve healthy aging goals. Care Navigators work with Older Adult Services and Behavioral Health Departments to provide comprehensive care.

• Assists clients to recognize strengths and values. Provides practical information, strategies and skills that the clients can use in a guided process.

• Assists and encourages participation in the development of an individualized, comprehensive plan towards healthy aging, and illness management and recovery.
• Assesses clients in their natural home environment and fosters the development of natural/social supports.
• Advocates for clients to ensure equity of treatment, opportunities, and access.
• Provides outreach and advocacy for aging and frail older adults, and some whom experience severe mental illness
• Maintains communication with clients and ancillary service providers to assure services.
• Works closely with entire JFS Akron team to arrange and coordinate needed services.
• Maintains appropriate electronic health record and completes billing documentation in a timely fashion that adheres to accrediting service standards.
• Provides other mental health services as assigned according to licensure and state standards.
• Provides social service coordination in HUD buildings.
• Drives his/her personal auto between assignments, and transport clients as required. May use Akron agency van if available.
• Demonstrates awareness of, follows all safety policies and procedures, and adheres to all recommended safe work practices. Also contributes to the control of hazards in specified area of responsibility.
• Performs other duties or special projects as required or as assigned.

Education and Experience:

• Bachelor's degree in counsing, psychology, social work or related field
• Master's degree preferred and licensure a plus
• Two-five years of relevent social services expereince with an emphasis on older adults or aging populatons

Licensure:

• Maintain both a valid Ohio Drivers License, and a driving record that meets JFSA's insurance company standards (e.g. less than 2 incidents over a 3 year period)
• Maintain personal auto liability insurance at the level that the Agency and the insurance carrier requires
• Must have a reliable personal automobile available daily
