Job Overview

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. Responsibilities

  • Provide direct patient care, including administering medications and treatments as prescribed by physicians.
  • Conduct patient assessments, monitor vital signs, and document medical histories accurately.
  • Assist with daily living activities and support patients with mobility and personal hygiene.
  • Collaborate with healthcare professionals to develop and implement individualized care plans.
  • Educate patients and families on health management, medication administration, and post-discharge instructions.
  • Maintain accurate medical records in compliance with HIPAA regulations and facility policies.
  • Participate in infection control measures and ensure a safe environment for patients.
  • Assist in triaging patients and managing acute pain as necessary.
  • Support the nursing team in various tasks, including IV infusion, catheterization, and wound care.

Requirements

  • Valid LPN license in the state of practice.
  • Experience in critical care, long-term care, or assisted living preferred.
  • Familiarity with EMR/EHR systems such as Epic, Cerner, or Athenahealth is advantageous.
  • Knowledge of medical terminology, ICD coding (ICD-9/ICD-10), and basic life support protocols.
  • Strong patient assessment skills with the ability to recognize changes in patient conditions.
  • Experience working with individuals with developmental disabilities or dementia is a plus.
  • Proficiency in medication administration, phlebotomy, venipuncture, and specimen collection/processing.
  • Excellent communication skills and the ability to work collaboratively within a healthcare team.
