LPN Home Health Private Duty

Job Summary

As an LPN, you will work under the direction of an RN and be responsible for providing comprehensive nursing care to patients in their home. You will work collaboratively with physicians and other healthcare professionals to ensure the highest standards of patient care are met.

Duties
- Provide direct patient care, including administering medications, Gtube feedings Trach Care, monitoring vital signs, and assisting with daily living activities.
- Develop and implement individualized care plans based on patient needs and medical history.
- Collaborate with healthcare professionals to coordinate care and ensure comprehensive treatment.
- Educate patients and their families about health management, medication administration, and post-treatment care.
- Maintain accurate documentation of patient progress and changes in condition.
Requirements
- Valid LPN nursing license in the state of Ohio.
- Experience in assisted living, nursing home settings, or acute care environments is preferred.
- Strong communication skills to effectively interact with patients, families, and healthcare teams.
- Ability to work independently while demonstrating critical thinking skills in patient assessment and decision-making.
- Compassionate demeanor with a strong commitment to providing high-quality patient-centered care.

Job Types: Full-time, Part-time, PRN
Pay: $25-27 per hour
Expected hours: 10-40 hours per week
