We are currently seeking a qualified facility LPN or RN – MDS Nurse to join our team at Peak Resources Wilmington.

Typical Work Schedule is: Mon-Fri /8:00am-4:30pm

Role Responsibilities

Coordinates and facilitates the timely and accurate completion of both the RAI and Care Management process from admission to discharge in accordance with Company P&P's, State, and Federal guidelines, and all other entities as appropriate. The MDS Nurse- LPN will work under the supervision of an RN.

Requirements:

Required Skills, Education, Licensure, and/or Certifications

  • Current, unencumbered, active license to practice as an LPN in NC.
  • Knowledge and experience of the RAI process required
  • Proficient computer skills
  • Excellent interpersonal and communication skills
  • Must not pose a direct threat to the health or safety of other individuals in the workplace
  • Fulltime MDS Nurse will be required to become RAC certified and maintain recertification every 2 years

Preferred Experience

  • One to two (1-2) years nursing experience in a hospital or nursing facility
  • One to two (1-2) years of MDS experience
  • Knowledge of Medicare and Medicaid processes
