We are looking for a Licensed Practical Nurse for our Full Time ALL shifts. We offer a competitive pay range of $28.00- $30.00 an hour including weekend and night shift differentials.

Reports To: Director of Nursing

Location: River City Rehab Center, 15480 Max Leggett Parkway, Jacksonville, FL 32218

Status: Non-exempt

SUMMARY

Provides residents with nursing services and administers medications and treatment in accordance with physicians' orders within state licensure regulations.

PRIMARY DUTIES AND RESPONSIBILITIES

• Assists in maintaining a physical, social and psychological environment in the best interests of residents.
• Monitors residents within state licensure regulations; may supervise and direct resident assistants where allowed by state licensure regulations.
• Assists in writing service plans based on resident needs.
• Obtains and administers medication and treatments as prescribed by the physician. Documents and reviews medication and treatment sheets for accuracy and compliance with physician orders, professional standards, federal and state regulations and company policy.
• Monitors responses to treatment plans. Possesses knowledge of medication interactions and notifies Resident Care Coordinator/HeaIth Care Coordinator of any occurrences.
• Counts controlled substances with an off-going nurse or resident assistant; secures and corrects reconciliation of controlled substances during shift.
• Assures cleanliness and organization of medicine supply room and security of discontinued medicine in accordance with federal and state regulations and company policy.
• Assists with discharge process relevant to medication/treatment orders.
• Possesses knowledge of OSHA regulations and infection control standards.
• Follows policies and procedures to ensure resident safety.
• Encourages teamwork through cooperative interactions with co-workers.

QUALIFICATIONS

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION AND/OR EXPERIENCE

High school diploma or general education degree (GED); and three to six months related experience preferred and/or training in direct service with older adults, or equivalent combination of education and experience. In accordance with state law, may need to possess current state certification and follow regulations to maintain license currency.

Knowledge of dementia particularly of the Alzheimer's type is helpful.

LANGUAGE SKILLS

Ability to read and comprehend simple instructions, short correspondence and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one and small groups situations to residents, residents' families, and other employees of the company. Ability to effectively communicate in English.

MATHEMATICAL SKILLS

Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, and volume. Ability to apply concepts of basic algebra and geometry.

REASONING ABILITY

Ability to solve practical problems and deal with a variety of concrete variables in standardized exists. Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form.

CERTIFICATES, LICENSES, REGISTRATIONS

Basic CPR/First aid training

Fire safety training

Current nurse (LPN/LVN) licensure issued by appropriate state licensing agency.

Current nurse (RN) licensure issued by appropriate state licensing agency.

WORK ENVIRONMENT

While performing the duties of this job, the employee is frequently exposed to bloodborne pathogens. The employee is occasionally exposed to fumes and toxic caustic chemicals. The environment is characteristic of a facility in which healthcare is provided to the frail elderly. The noise level is usually moderate. May be required to travel for group outings.
