Position Summary:

The Health & Wellness Coordinator expands access to physical and mental health resources for survivors of domestic violence participating in YWCA Utah's residential and community-based programs. This role connects survivors with individualized and group therapy, community supports, and additional health and wellness services that promote holistic, trauma-informed healing.

The Coordinator cultivates and maintains strong community partnerships, designs innovative resource solutions for underserved populations, and oversees accurate data collection to meet Intermountain Community Care Foundation (ICCF) grant requirements. Working closely with internal teams, external providers, and program participants, this position plays a key role in supporting comprehensive wellness throughout the healing journey.

Essential Duties & Responsibilities

Grant Compliance and Reporting

  • Ensure full compliance with ICCF grant requirements, including annual reporting, accurate data tracking, and monitoring key program outcomes.
  • Maintain timely, detailed documentation of participant information and services in ETO.
  • Stay informed on current health, safety, and wellness protocols.

ICCF Grant Roles

  • Serve 170 survivors annually (510 total).
  • Increase access to individual therapy by 25%.
  • Double the number of children accessing therapy.
  • Increase group therapy participation by 11%.
  • Increase referrals to community-based therapy and support groups by 60%.

Program Activities

  • Coordinate the weekly on-site therapy schedule.
  • Connect participants to bilingual and culturally responsive therapy services, including Spanish-speaking providers.
  • Facilitate access to child-focused therapies such as play and art therapy.
  • Support coordination of on-site Peer Support Groups.
  • Arrange transportation assistance for off-site therapy appointments.
  • Administer participant surveys to assess mental health and coping outcomes.
  • Gather and document participant success stories for reporting and evaluation.

Community Partnerships

  • Maintain strong, collaborative relationships with healthcare, behavioral health, and social service partners.
  • Develop new partnerships that broaden access to health and wellness resources.
  • Coordinate on-site mobile medical clinics to meet diverse participant health needs.

Client Services

  • Provide ongoing health navigation, resource referrals, and follow-up support.
  • Manage scheduling and logistics for participant services.
  • Serve as a liaison between participants and healthcare providers to ensure continuity of care.
  • Develop responsive, creative solutions that address the unique wellness needs of underserved populations.

Additional Duties and Responsibilities:

  • Uphold and act in accordance with the mission of the organization, vision, values and strategic direction of the organization.
  • Demonstrate commitment to understanding and using Sanctuary Model principles, language, and tools in daily work.
  • Model YWCA community standards and encourage program participants and other staff to follow them.
  • Understand and follow all established organizational and program related policies, procedures and protocols.
  • Keep all records necessary to ensure that work is conducted in accordance with agency, contractual, and legal requirements. Records are expected to be complete, accurate, and timely.
  • Satisfactorily perform all duties outlined in this job description.
  • Maintain all certifications and licensing related to role, program licensing, and grant standards including but not limited to: CPR, First Aid, Food Handlers Permit, LCSW/CSW/SSW, etc.
    Attend all required staff meetings, individual and/or group supervision, agency-wide staff and other trainings, as well as any other meetings and activities as requested.

  • Ability to work respectfully with people of diverse ages, races, ethnicities, nationalities, socio-economic levels, beliefs and lifestyles.
    Must be able to pass Department of Health and Human Services background check, meet I-9 requirements, and, if driving on agency business, a valid Utah driver's license, automobile insurance and motor vehicle record.

Physical/Sensory Demands:

Work is generally performed in a safe and comfortable office environment. While performing the duties of this job, the employee is regularly required to: sit for long periods of time; use computer to enter data; use telephone to converse with customers and employees; walk and stand for long periods of time; perform repetitive wrist, hand and/or finger movement. Travel to attend local conferences, training, outreach and other events occurs routinely.
