The Investigator & Incident Coordinator is responsible for the coordination, investigation, and resolution of reportable incidents across assigned RHD programs. Working under the direction of the Director of Incident Management & Investigations (PA or Regional), the Investigator ensures that all incidents are documented accurately, investigated thoroughly, and resolved in a manner that aligns with regulatory requirements, RHD policy, and best practices.

This position plays a critical role in supporting a culture of safety, integrity, and continuous improvement. The Investigator collaborates with program staff, leadership, and external stakeholders to promote compliance and ensure proper response to allegations of abuse, neglect, exploitation, and other reportable events. The Investigator will serve as a resource and mentor to program-level incident reporters, ensuring staff are trained and supported in documentation and resolution.

The PA-based role will focus primarily on Pennsylvania programs and their corresponding regulatory agencies (e.g., ODP, OMHSAS, OLTL), while the regional role will serve a broader, multi-state area.

Section 3: Major Responsibilities

  • Receive, review, and manage reportable incidents across assigned programs in alignment of Funders Regulations.
  • Conduct thorough investigations in accordance with state and federal guidelines and RHD policies.
  • Interview staff, individuals served, families, and witnesses while ensuring trauma-informed practices.
  • Collaborate with program leadership and HR on personnel-related concerns arising from investigations.
  • Ensure all reports and supporting documentation are completed timely and submitted to oversight agencies as required.
  • Participate in incident review committees and contribute to follow-up planning.
  • Serve as a subject matter expert on incident documentation and regulatory reporting.
  • Assist with training staff in incident management systems and documentation procedures.
  • Monitor progress on corrective action plans and ensure follow-through by responsible parties.
  • Maintain confidentiality and professionalism throughout all investigative processes.
  • Serve as liaison with external regulatory bodies as assigned.
  • Travel to program sites as needed to conduct in-person investigations.
  • Perform other duties as assigned.

Section 6: Job Qualifications and Competencies

Job Qualifications:

Education and Experience:

  • Bachelor's degree required; preferred in human services, criminal justice, or related field.
  • At least 2 years conducting investigations in a behavioral health or IDD setting.
  • Familiarity with state-specific systems and agencies (e.g., HCSIS, EIM, ODP) strongly preferred for PA role.
  • Valid driver's license with clean MVR and willingness to travel.
  • Satisfactory background check required.
