About Care Assurance Aging Solutions

Care Assurance is a Georgia Medicaid waiver case management agency serving CCSP/SOURCE/EDWP participants with excellence, compassion, and high accountability.

We are expanding our nurse network and seeking professional, reliable Licensed Practical Nurses (LPNs) to conduct home-based assessments, reassessments, and care plan reviews in their local region.

This is NOT facility nursing.

No med passes.

No heavy lifting.

No 12-hour shifts.

No weekends.

HYBRID documentation.

Meaningful client-facing work.

Flexible and calm compared to traditional bedside roles.

All documentation is reviewed and signed off by our supervising RN per GA Nursing Practice Act.

Position Summary

The LPN Field Nurse performs:

  • Initial Assessments
  • Annual Reassessments
  • 90-Day Reviews
  • 30-Day Reviews
  • In-home functional assessments
  • Environmental and safety scans
  • Collection of required client signatures
  • Care plan review with client/family
  • Paper packet completion inside the home
  • Electronic documentation after the visit
  • Prompt communication with the Lead Care Coordinator

You are assigned clients only in counties closest to your home.

Compensation (Per Visit Pay Structure)

This is a per-visit contractor role. Hourly range listed reflects visit equivalents.

Per-Visit Rates:

  • Initial Assessment: $75
  • Annual Reassessment: $65
  • 90-Day Review: $50
  • 30-Day Review: $40

Travel Policy:

  • No mileage reimbursement for first 30 miles round trip
  • Mileage over 30 miles reimbursed at $0.35/mile
  • Nurses are assigned local clients to minimize travel

Schedule & Workload

  • Monday–Friday (weekends are acceptable based on mutually agreed terms between the client and nurse)
  • Varies (preferred during business hours)
  • Choose your own visit times
  • 4–10 visits/month depending on caseload and region
  • Work as much or as little as you prefer
  • No mandatory nights, weekends, or holidays

Requirements

Required:

  • Active Georgia LPN License
  • Reliable transportation
  • Smartphone and ability to upload documentation
  • Professional phone presence
  • Strong communication skills
  • Ability to complete visit documentation within 24–48 hours

Preferred:

  • Senior care experience
  • Home health or hospice background
  • Community-based nursing experience
  • Familiarity with Medicaid Waiver programs (EDWP/CCSP/SOURCE)
  • Case management support experience
