This position is responsible for providing residents with quality care. Follow care plans, coordinate with the nursing team and report pertinent information to the immediate supervisor.

  • Maintain Resident privacy and confidentiality. Treat Residents with respect and dignity.
  • Report Resident complaints and comments to the appropriate person.
  • Participate and receive Resident report on Resident's condition.
  • Perform Resident care tasks as assigned.
  • Cooperate with team members to provide complete care.
  • Answer residents' call lights promptly and courteously.
  • Provide resident hygiene, grooming and dressing needs.
  • Make beds and keep Resident room/belongings orderly.
  • Follow mealtime procedures including tray passing, setup, Resident positioning and assisting/feeding the Residents.
  • Document food and fluid intake and output.
  • Measure and document temperatures and weight accurately.
  • Position and reposition, transfer and transport Residents safely.
  • Use patient equipment safely.
  • Assist Residents with elimination needs and record it appropriately.
  • Assist with Resident admission and discharge needs.
  • Provide postmortem care.
  • Adhere to standard and universal precautions including practicing proper hand washing. Handling clean and dirty linen properly.
  • Float to other units as needed.
  • Provide individualized attention, which encourages each resident's ability to maintain or attain the highest practical physical, mental, and psychosocial well-being.
  • Maintain Resident privacy and confidentiality.
  • Fully understand all aspects of residents' rights, including the right to be free of restraints and free of abuse. Responsible for promptly reporting to the charge nurse or administrative staff incidents or evidence of resident abuse or violation of residents' rights.
  • Maintain the comfort, privacy, and dignity of each resident in the delivery of services to them.
  • Interact with residents in a manner that displays warmth and promotes a caring home like environment.
  • Assist in maintaining a safe, neat, and clean environment; report environmental deficiencies to the charge nurse such as lighting or equipment problems.
  • Protect the personal belongings of each resident. Promptly report missing items according to established policy.
  • Observe residents for changes in condition or behavior and promptly report these changes to appropriate licensed nursing personnel.
  • Perform various tasks assigned by the charge nurse including vital signs, residents' weights, etc.
  • Communicate and interact effectively and tactfully with the resident, visitors, families, peers, and supervisors.
  • Complete certified nursing assistant records documenting care provided or other information in keeping with department policies.
  • Committed to working with residents and families to develop and implement strategies that promote person-centered care.
  • Applies "5 Star" customer service approach in all aspects of job.
  • Actively supports Carleton-Willards's commitment to providing "5 Star" service on all quality objectives, with an emphasis on continual improvement.
  • Adheres to all company policies, procedures, and standards of conduct.
  • Reports to the appropriate managers any known or suspected violations of policy and procedures, regulations or standards of conduct.
  • Performs additional duties as assigned.

  • Graduation from an approved Nurse Aide Training Program.

  • 1-2 years' experience preferable in long-term care.
  • Ability to read, write and communicate in English.
  • Massachusetts Nurse Aide Certification.
  • Team Player.

  • 80% of the shift spent standing or walking.

  • Ability to perform walking rounds throughout the facility.
  • Exposed to infections and odors, including blood and/or body fluids.
