In this role, you will provide community-based mental health support through individualized planning, skill-building, and evidence-based interventions. You will partner closely with clients, families, and other providers to deliver coordinated, high-quality services tailored to each person's needs. Your work will focus on fostering resilience, connecting clients with community resources, and promoting meaningful progress in their daily lives.

Key Responsibilities:
· Provide individualized, community-based mental health services that empower clients to build skills, strengthen support systems, and progress toward personal goals.
· Collaborate with clients, families, and support networks to develop and implement personalized treatment plans.
· Deliver evidence-based interventions using approaches such as motivational interviewing, trauma-informed strategies, positive reinforcement, CBT- and DBT-based skills, and behavioral techniques.
· Coordinate care and maintain strong partnerships with primary care providers, psychiatrists, schools, and community organizations.
· Maintain accurate, timely documentation that reflects client progress and meets organizational standards.
· Participate in ongoing professional development, training, and staff education activities.
· Contribute to quality improvement processes through data collection, analysis, and reporting.
· Foster strong, respectful working relationships with clients, families, teachers, counselors, and other professionals.
· Prepare for and facilitate PSR group sessions, ensuring alignment with treatment plan goals.
· Perform additional duties or responsibilities as assigned.

Associate of Applied Science in Behavioral Health Support degree from an approved institution

Work experience in a related human services field may substitute for educational requirements. Related fields may include direct support such as case management, residential support, developmentally disabled support, vocational rehabilitation, and more. Four years of qualifying experience minimum or a combination of education and experience equaling four years required.

Qualified Addictions Professional (QAP) - CADC, CRADC, CRAADC, CCJP, RADC-P, RADC, CCDP, CCDP-D
Additional Qualifications:
· Must be at least twenty-one (21) years of age to operate a company-owned vehicle and at least twenty-three (23) years of age to operate a company-owned fifteen (15) passenger van.
· Current driver's license, acceptable driving record, and current auto insurance.
· Must have reliable transportation to transport clients in personal vehicle.
· Adequate written communication skills to complete required documentation.
· Ability to provide high-level performance while meeting complex client needs.
· Preferred skills include strong communication, problem solving, ability to follow direction, multitasking, and establishing effective relationships with clients and families.
· Must attain trainings required to support professional development and meet DMH standards.
Physical Requirements:
ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, pull, or move objects. Repetitive movements of hands, fingers, and arms for typing and/or writing during the work shift. Sedentary work involves mostly sitting, with occasional walking or standing.
