Job Overview

We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting clients by assessing their needs, developing care plans, and coordinating services to ensure they receive the best possible support. Responsibilities

  • Conduct comprehensive assessments of clients' needs and circumstances.
  • Develop and implement individualised care plans in collaboration with clients and relevant stakeholders.
  • Coordinate services and resources to ensure clients receive appropriate support.
  • Maintain accurate and up-to-date case records and documentation.
  • Provide ongoing support and follow-up to clients, ensuring their needs are met effectively.
  • Liaise with other professionals, agencies, and community resources to facilitate client access to services.
  • Monitor client progress and adjust care plans as necessary based on feedback and changing circumstances.
  • Uphold confidentiality and adhere to ethical standards in all interactions.

Requirements

  • Proven analysis skills with the ability to assess complex situations effectively.
  • Strong customer service orientation, demonstrating empathy and understanding towards clients.
  • Previous administrative experience is essential for managing case documentation efficiently.
  • Excellent organisational skills with the ability to prioritise tasks in a fast-paced environment.
  • A relevant qualification in social work, psychology, or a related field is preferred but not mandatory.
