Overview:

We are seeking a dedicated Nurse Manager to join our team. As a Nurse Manager you will posses a strong background in nursing and management, overseeing the day-to-day operations of our Home Care Agency.

Duties:

  • Ensure compliance with healthcare regulations and standards
  • Conduct initial assessments and develop individualized care plans for patients
  • Develop and implement policies and procedures to improve patient care
  • Monitor and evaluate patient progress adjusting care plans as necessary
  • Collaborate with medical providers to enhance patient outcomes
  • Document patient assessments, interventions, and outcomes accurately and timely
  • Maintain accurate and up-to-date medical record as needed
  • Perform comprehensive nursing assessments and ongoing patient evaluations
  • Monitor vital signs, symptoms, and changes in patient condition
  • Develop, implement, and update individualized care plans
  • Provide wound care, injections, catheter care, and other skilled nursing services as needed
  • Educate patients and family members on disease management, medications, and safety
  • Coordinate care with caregivers, therapists, and other healthcare professionals
  • Maintain accurate, timely documentation in compliance with agency and regulatory standards
  • Ensure adherence to infection control, safety, and privacy (HIPAA) guidelines

Experience:

The ideal candidate should possess the following skills and experience:

  • Previous experience as a registered nurse
  • Proficiency in ICD-10 coding and documentation
  • Knowledge of utilization review processes
  • Familiarity with Wellsky or AxisCare or similar patient record systems
  • Background in emergency medicine or critical care
  • Ability to perform sutures when necessary
