Job Overview:

Our medical practice is seeking a motivated and compassionate Virtual Chronic Care Management (CCM) Specialist to join our care coordination team. This position offers the opportunity to work from home while helping patients manage their chronic conditions through proactive communication, education, and coordination of care.

The role is ideal for a Certified Medical Assistant (CMA) or Licensed Practical Nurse (LPN) who is confident, independent, and looking for something different than a traditional office role.

You will be trained as a CCM Champion using Dulcian Health, which integrates seamlessly with our Athena (athenaOne) electronic health record (EHR) to streamline care management activities.

Compensation

  • Starting pay: $16.00/hour for the first 90 days
  • Evaluation at 90 days: Performance and patient engagement/success metrics will be reviewed
  • Opportunity for raise: Based on demonstrated productivity, patient engagement, and contribution to program growth and profitability

Key Responsibilities

  • Identify and outreach to eligible patients for enrollment in the CCM program
  • Explain program benefits and obtain patient consent for participation
  • Conduct monthly patient check-ins, focusing on care coordination, education, and barrier resolution
  • Maintain accurate documentation of all patient interactions, goals, and care plans within Athena and Dulcian Health
  • Collaborate closely with providers, nurses, and office staff to ensure seamless continuity of care
  • Track and report on patient engagement and outcomes
  • Participate in training and regular team meetings (virtual and in-office as needed)

Qualifications

  • Certified Medical Assistant (CMA) or Licensed Practical Nurse (LPN) (required)
  • Previous experience in primary care or chronic disease management preferred
  • Strong communication and interpersonal skills
  • Self-motivated, organized, and able to work independently from home
  • Comfortable with technology, including EHR systems
  • Experience with Athena (athenaOne) EHR preferred
  • Passion for patient care and improving health outcomes

Training & Support

  • Onboarding and initial training will occur in-office, covering:
  • Athena EHR basics
  • Dulcian Health CCM software
  • Chronic Care Management expectations and workflows
  • Ongoing support and collaboration with the clinical and administrative team

Work Environment

  • This position can be fully remote after training, though some in-office work may be available or required for training and meetings.
  • Must have a quiet, secure workspace and reliable internet connection.
