Job Summary

The Case Manager reports to the Case Manager Supervisor and is responsible for providing ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual client needs; enhancing the quality of client management and satisfaction through the integration of case management, utilization review, and goal planning; having accountability for the care, coordination and goal planning of assigned clients; and performing other related duties as assigned.

Knowledge, Skills, and Abilities

  • Knowledge of the principles of growth and development over the life span.
  • Knowledge of policy, principles, and practice of social work and/or psychology, individual and group behaviors, socioeconomic factors, and their relationship to mental health. Has resources at hand for local, state, or national needs for individuals with substance use and/or mental health concerns.
  • Knowledge of managed healthcare programs, including Tenn Care, eligibility issues, and insurance reimbursement.
  • Knowledge of guidelines and methods used for applying for disability and for accessing vocational rehabilitation services.
  • Knowledge of systems involves in the provision of healthcare.
  • Ability to provide excellent customer service and support.
  • Ability to interact professionally with persons from diverse cultural, socioeconomic, education, racial, ethnic, and professional backgrounds effectively.
  • Ability to work with managers, co-workers, members of the public, and professional groups effectively.
  • Ability to organize and prioritize workload and meet deadlines effectively.
  • Ability to communicate effectively.
  • Ability to consistently demonstrate a positive/proactive customer service attitude.
  • Ability to maintain ethical behaviors exemplary of quality public service and fair standards, inclusively, among all employees and members of the public.
  • Ability to work as an effective team member, function independently, and exercise sound judgment and initiative.
  • Ability to develop and maintain positive interpersonal relationships.
  • Ability to maintain confidentiality.
  • Ability to work within a regional and state service network and interact with other healthcare professionals.
  • Ability to utilize computer programs to prepare reports and assist with grant applications.
  • Working knowledge of Microsoft Office and Electronic Health Records.
  • Ability to organize and maintain an accurate filing system. Ability to work independently with minimal supervision.

Required Qualifications

  • Bachelor's degree in a relevant field and two (2) years of experience

Or

  • An equivalent combination of education, training, and experience

Preferred Qualifications

  • One year of experience working with addiction and/or mental health diagnoses
