Job Summary
We are seeking a highly skilled and compassionate Family Nurse Practitioner to join our healthcare team. The ideal candidate will possess extensive clinical expertise across various specialties, including primary care, behavioral health, and specialty areas. This role involves providing comprehensive patient assessments, managing acute and chronic conditions, performing procedures such as suturing and IV infusions, and utilizing advanced EMR/EHR systems like Epic, Cerner, and eClinicalWorks. The Family Nurse Practitioner will deliver high-quality care in diverse settings including outpatient clinics, inpatient units, telehealth platforms, and assisted living facilities.
Responsibilities
- Conduct thorough physical examinations and patient assessments across all age groups, including infants, toddlers, seniors, and individuals with developmental disabilities or memory care needs.
- Manage acute pain and chronic conditions utilizing evidence-based practices such as pain management protocols, dermal fillers, botulinum toxin treatments, esthetic laser treatments, and infection control measures.
- Perform procedures including suturing, catheterization (urinary and cardiac), venipuncture, IV insertion and infusion, spinal taps, gastric lavage, tube feeding management, sterile processing techniques, and airway management.
- Administer injections and medications safely while adhering to HIPAA regulations and medical documentation standards.
- Utilize advanced EMR/EHR systems such as Epic, Cerner, Athenahealth, and eClinicalWorks for accurate record keeping and clinical documentation.
- Coordinate discharge planning, case management, utilization review/management, and post-acute care planning to ensure seamless patient transitions across healthcare settings.
- Provide telehealth consultations leveraging telehealth experience to extend access to care for remote or underserved populations.
- Collaborate with multidisciplinary teams in hospital medicine units including ICU (Level II Trauma Center experience), PICU (Pediatric Intensive Care Unit), neonatal ventilator management, and emergency room settings with Level I Trauma Center experience.
- Engage in health coaching initiatives focusing on nutrition, disease prevention (e.g., diabetes management), family planning, and patient education tailored to individual needs.
- Ensure compliance with infection control protocols during specimen collection/processing and sterile techniques during procedures.
Experience
- Proven clinical experience in diverse healthcare environments such as hospitals (including trauma centers), outpatient clinics, nursing homes (including dementia care), hospice & palliative medicine settings.
- Demonstrated expertise in managing complex cases involving infectious disease care, occupational health issues including Workers' Compensation law compliance, neurodegenerative conditions like Alzheimer's disease or dementia care.
- Experience working with specialized populations such as children (including neonatal ventilator management), seniors (geriatrics), patients with developmental disabilities or memory impairments.
- Proficiency in performing diagnostic evaluations using laboratory testing (including phlebotomy) and medical imaging techniques; familiarity with ICD-9/10 coding for accurate billing.
- Skilled in acute care procedures such as airway management in ICU or emergency medicine contexts; familiarity with CPT coding for procedures performed.
- Knowledge of pharmacology related to psychopharmacology for behavioral health cases; experience administering injections; understanding of medication administration protocols.
- Ability to work effectively within outpatient or inpatient settings using EMR systems like eClinicalWorks or Cerner; experience with utilization review/management processes is a plus.
