PHYSICAL AND MENTAL REQUIREMENTS

Reports to the HCS Director and serves as the primary program contact for assigned clients in the HCS and TXHML Program. The Case Manager is ultimately responsible for assuring the delivery of quality services to each individual on their assigned caseload. Bilingual in English and Spanish is preferred

ESSENTIAL FUNCTIONS:

  • Conduct Annual Staffing's according to IPC renewal or transfer/intake date. This includes up-dating the disaster plan, demographics, Emergency back-up plan for host homes, conducting the environmental and hot water assessment (photographic evidence) Submit Staffing packet to HCS Program Director for review
  • Complete IDRC, IPC w/ corresponding IP's, and (ICAP every 3-years) (For each service provided &/or discontinued) according to PDP during Annual Staffing's, or as needed when revising services.
  • Review Utilization weekly, adjust units of services as needed.
  • Provide service unit up-dates to adjunctive service providers as needed
  • Conduct Quarterly review of each assigned client, including utilization, pending services/potential delays of service, etc. Share with Client/LAR via email, in person or USPS
  • Review Emails twice daily, respond and follow up by end of business day.
  • Review all HH Paper work for accuracy, and respond with receipt, review fire drills for accuracy. Print HH paper work and file in individual drop files, print fire drills submit to Q.A.D.
  • Audit/purge/file in Hard Copy Charts according to Audit year according to assigned case load.
  • Assist individuals/LAR as needed with services such as Title 19, CTA reimbursements, vendor assistance, i.e. Dentist, submit AA & Dental invoices to HCS Program Director for billing.
  • Answer/return ALL calls by end of business day. Check Voice Mails & clean out voice mails.
  • Maintain Electronic Drive, Physical Charts & drop files for assigned caseload according to Electronic Drive Order flow sheet located:
  • Keep outlook calendar up-dated at all times, to include scheduled staffing's, home visit's, quarterly reviews, and environmental. Share Calendar with HCS Program Director, Residential Coordinator, Quality Assurance Director, Executive Director, Nursing Mgr.

EDUCATION, SKILLS, AND ABILITIES

  • Associate's Degree or minimum 3 years of relevant work experience required; bachelors degree preferred
  • Experience leading/supervising individuals and teams is a plus.
  • Interest or desire in providing resources and support to individuals with intellectual and developmental disabilities.
  • Ability to communicate effectively in both oral and written modes
  • Ability to work with limited supervision and utilize independent judgment to plan, establish priorities, and conduct work activities effectively
  • Ability to maintain a high degree of confidentiality and professionalism in all aspects of the job, including displaying courtesy and sensitivity in all contacts and professionalism in all contacts and managing difficult situations effectively
  • Ability to coordinate with other staff, departments, volunteers, officials, agencies, organizations and the public
  • Must possess basic principles of office organization, operate a variety of office equipment and possess appropriate telephone techniques and etiquette.

  • Requires the ability to see and read a computer screen and printed material

  • Requires the ability to work in a normal seated position for extended periods of time
  • Requires the ability to enter data into a computer, operate standard office equipment, and use a telephone
  • Requires the ability to stay focused on a critical task while experiencing distractions or interruptions
