Job Title: Shelter Case Manager

SUMMARY: This is a full-time, salaried position responsible for overseeing shelter occupancy and providing individualized case management to shelter clients.

DIVISION: Full-Time Staff

REPORTS TO: Executive Director

Qualifications:

  • Completion of a Bachelor's Degree in one of the following areas or equivalent experience: Social Work, Psychology, Sociology, Family Studies & Human Services

  • Good written and verbal communication skills

  • Basic computer skills

-Previous experience in disability services

Requirements:

  • Must be able to complete work Monday thru Friday between the hours of 8am and 5pm.

  • Some weekend and evening hours may be required.

Salary & Benefits:

  • Starting salary is $49,500 -$53,000 per year

  • Paid vacation and sick leave

Primary Duties

Provide Individualized Case Management to Shelter Clients

  • Meet with all shelter clients within 24 hours of checking in and at least weekly thereafter to discuss their progress.
  • Assess client's housing needs and establish an appropriate case plan to meet these needs.
  • Evaluate the level of supportive services each client requires and respond appropriately.
  • Communicate all relevant client information and decisions to the Executive Director and other supportive staff, as necessary.

Manage Shelter Occupancy

  • Respond to all requests for emergency housing.
  • Determine occupancy arrangements within the shelter.
  • Communicate bed turn-over needs to support staff or facilitate bed turn-over daily.
  • Enforce and manage the shelter house rules and all shelter policies and procedures.

Provide Rapid Re-Housing Services

  • Meet with potential participants seeking rapid re-housing assistance.
  • Develop housing case plans as directed for eligible participants.
  • Distribute financial assistance per agency policy based on availability of funds.
  • Perform all required and necessary inspections of housing units.
  • Provide follow-up housing stabilization case management for eligible participants.
  • Adhere to all federal regulations while performing rapid re-housing activities.

Maintain Client Files and Prepare Reports

  • Maintain complete files for each client documenting all interactions and relevant information, including all federal documentation requirements.
  • Input relevant data into the HMIS data system in accordance with agency policy.
  • Manage data entry issues and run reports for federal or grant requirements.

Provide Supportive Services to the Executive Staff

  • As assigned, perform supportive duties that may include answering the phone, running errands, managing the facility in executive staff's absence.
  • Assist Executive Director as requested.
  • Take after-hours calls and respond to shelter emergencies when asked.
  • Work with other staff to maintain organization and cleanliness of facility.

  • Perform duties in an ethical manner, maintaining professional boundaries.
  • Enforce all policies equitably and in compliance with applicable laws regarding discrimination, civil rights, and mandatory reporting of abuse.
  • Follow all agency policies and procedures.
  • Maintain a safe and respectful working environment for both clients and other employees.

· Must be able to cope with stress brought about by dealing with a case load of up to 25 clients with diverse needs and barriers.

· Must be able to exercise independent judgment in a fast-paced environment.

· Must be able to lift objects weighing up to 30 pounds on occasion.

· Must be able to walk up and down stairs without assistance.

Job Type: Full-time

Pay: $49, $53,000.00 per hour
