About SocialRx Inc
SocialRx Inc (Art Pharmacy) is a digital health company reimagining behavioral and social health through social prescribing and community-centered care. We partner with healthcare, education, and workforce management partners to foster purpose, belonging, and well-being. With contracts across AZ, CA, CT, GA, NC, NY, MA, NH, and WI, we're scaling nationally to deliver measurable, meaningful impact.
Overview
We're looking for a thoughtful, relationship-driven Partner Success Manager to support and strengthen Art Pharmacy's growing network of partners across education, health systems, health plans, government entities, and community organizations. This role is focused on the post-implementation lifecycle, ensuring each client feels supported, engaged, and successful long after launch. You'll serve as the primary point of contact for many clients, fostering long-term relationships that drive engagement to ultimately drive referrals. The ideal candidate is an organized individual who can build strong relationships and think creatively. You're as comfortable developing outreach plans and engagement reports as you are leading client check-ins or spotting opportunities for partnership growth. Occasional travel may be required for client meetings, site visits, and community events.
What You'll Do
Client Relationship Management
- Serve as the day-to-day contact for a portfolio of client accounts across healthcare, education, and community sectors.
- Build strong, trusted relationships with key stakeholders and decision-makers.
- Ensure each client's ongoing engagement, satisfaction, and success post-implementation.
- Coordinate and lead regular partnership check-ins, reviews, and feedback sessions.
Retention, Referral & Engagement
- Conduct and creatively strategize ways to drive referrals of members from prescribing partners.
- Monitor client activity and performance metrics to identify opportunities for additional referrals and deeper engagement.
- Share insights, best practices, and recommendations to help clients achieve their goals.
- Collaborate with internal teams to strengthen client onboarding, referral performance, and engagement strategies.
Collaboration & Reporting
- Maintain accurate account records, communications, and engagement metrics.
- Take detailed notes and provide updates to the Head of Strategic Partnerships to support the success of the client relationships.
- Partner cross-functionally with implementation, care coordination, and business development teams to ensure a seamless client experience.
What We're Looking For
- 2–3 years of experience in client success, partnerships, or account management, ideally within healthcare, education, or community sectors.
- Experience in the mental health field.
- We are particularly interested in individuals with clinical experience and/or a behavioral health license.
- Exceptional communication and relationship-building skills, you build trust and credibility quickly.
- Strong organizational skills and attention to detail, with experience managing multiple client accounts simultaneously.
- Comfort with data tracking and performance reporting; familiarity with CRM systems preferred.
- A collaborative, solutions-oriented approach with the ability to anticipate client needs.
- Willingness to travel occasionally for client meetings, events, and partner engagements.
Why You'll Love This Role
- You'll help our clients create real-world impact through non-clinical, community-based care.
- A mission-driven, collaborative culture that values creativity, curiosity, and strong relationships.
- Opportunity to shape the client experience at a growing, purpose-driven organization.
- Dynamic work that blends relationship management, strategic thinking, and measurable outcomes.
