Overview

We are seeking a dedicated and skilled Physical Therapist to join our healthcare team. The ideal candidate will have a strong background in patient care and a comprehensive understanding of physiotherapy principles. This role involves working with patients across various settings, including inpatient, outpatient, and home health environments. The Physical Therapist will be responsible for developing individualized care plans, implementing treatment strategies, and facilitating discharge planning to ensure optimal recovery for patients.

Duties

  • Conduct thorough assessments of patients to determine their physical therapy needs.
  • Develop and implement personalized treatment plans based on patient evaluations and goals.
  • Utilize various physiotherapy techniques, including trigger point therapy and orthopedic rehabilitation.
  • Provide patient education on exercises, home care strategies, and injury prevention.
  • Collaborate with interdisciplinary teams to coordinate comprehensive patient care.
  • Maintain accurate documentation of patient progress in EMR systems.
  • Monitor patient outcomes and adjust treatment plans as necessary.
  • Engage in discharge planning to facilitate smooth transitions from inpatient to outpatient or home care settings.
  • Work with pediatric, geriatric, and acute care populations as required.

Experience

Candidates should possess the following qualifications:

  • Strong knowledge of anatomy, physiology, and medical terminology.
  • Experience in various care settings including hospital, clinic, outpatient, and home health environments.
  • Familiarity with discharge planning processes and the development of effective care plans.
  • Previous experience working with diverse populations such as pediatrics and geriatrics is preferred.
  • Proficiency in using EMR systems for documentation and patient management.
