Overview

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Adult Day Health Center team. The ideal candidate will provide high-quality nursing care to our clients, ensuring their health and well-being in a supportive environment. This role requires strong clinical skills, a commitment to patient care, and the ability to work collaboratively with a diverse team.

Duties

  • Administer medications and treatments as prescribed by physicians.
  • Conduct patient assessments and monitor vital signs.
  • Assist with daily living activities and personal care for clients.
  • Collaborate with healthcare professionals to develop and implement individualized care plans.
  • Maintain accurate medical records and documentation in compliance with HIPAA regulations.
  • Provide education and support to patients and their families regarding health management.
  • Participate in case management and discharge planning as needed.
  • Ensure a safe and clean environment for all clients.

Skills

Candidates should possess a variety of skills, including but not limited to:

  • Experience with Epic, Cerner, or other EMR/EHR systems.
  • Critical care experience in settings such as ICU, Level I or II trauma centers.
  • Proficiency in IV infusion, medication administration, and catheter care.
  • Knowledge of medical terminology, ICD-9/ICD-10 coding, and phlebotomy techniques.
  • Familiarity with geriatrics, senior care, Alzheimer's care, memory care, and dementia care.
  • Experience working with individuals with developmental disabilities or in hospice care.
  • Strong patient assessment skills and the ability to perform physical examinations.
  • Ability to manage acute pain, including experience with pain management techniques.

Pay: $ $30.00 per hour
