Overview
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Adult Day Health Center team. The ideal candidate will provide high-quality nursing care to our clients, ensuring their health and well-being in a supportive environment. This role requires strong clinical skills, a commitment to patient care, and the ability to work collaboratively with a diverse team.
Duties
- Administer medications and treatments as prescribed by physicians.
- Conduct patient assessments and monitor vital signs.
- Assist with daily living activities and personal care for clients.
- Collaborate with healthcare professionals to develop and implement individualized care plans.
- Maintain accurate medical records and documentation in compliance with HIPAA regulations.
- Provide education and support to patients and their families regarding health management.
- Participate in case management and discharge planning as needed.
- Ensure a safe and clean environment for all clients.
Skills
Candidates should possess a variety of skills, including but not limited to:
- Experience with Epic, Cerner, or other EMR/EHR systems.
- Critical care experience in settings such as ICU, Level I or II trauma centers.
- Proficiency in IV infusion, medication administration, and catheter care.
- Knowledge of medical terminology, ICD-9/ICD-10 coding, and phlebotomy techniques.
- Familiarity with geriatrics, senior care, Alzheimer's care, memory care, and dementia care.
- Experience working with individuals with developmental disabilities or in hospice care.
- Strong patient assessment skills and the ability to perform physical examinations.
- Ability to manage acute pain, including experience with pain management techniques.
