POSITION SUMMARY

Under the direction of the Director of Clinical Operations, the Clinical & Community Support Assessor will provide case coordination and complete clinical assessments for individuals and families within the home, community, and office setting.

EDUCATION AND EXPERIENCE

  • Licensed Counselor or Social Worker with one of the following licenses: LPC, LCSW, LMFT with minimum of 5-year post-graduate experience
  • Previous experience in a clinical/therapeutic role in the healthcare field or clinical environment (Community-Based/Social Services experience)
  • Previous experience providing clinical assessment & therapy services for Department of Family and Children Services (DFCS) is a plus
  • 6 months experience with a Georgia core provider or Agency providing in-home services is a plus

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Build relationship and collaborate with case managers, contractors, and community partners to ensure effective treatment and case collaboration
  • Perform clinical assessment services to include (Substance Abuse, Domestic Violence, Parental Fitness, Trauma, Comprehensive Client and Family Assessments, Bonding and Attachment). Please note other trainings may be required to complete certain assessments
  • Support agency training and development needs
  • Facilitate clinical and support training for agency awareness and development
  • Providing support to the clinical team, including interns
  • Grow and build assessment team
  • Coordinate clinical services, including assigning cases, and requesting services
  • Monitor clinical assessment referrals to ensure that services are completed efficiently and timely
  • Assist with interviews and conduct orientation and training on clinical assessment policies and procedures
  • Reviewing clinical documentation to ensure that services meet agency guidelines and expectations
  • Must be willing to commute and see clients in their home and community, as well as in-office
  • Document & maintain case notes in the online electronic health record system (eCase Notes)
  • Maintain excellent communication and effective working relationships with other agency staff
  • Uphold current and developing agency processes to ensure a standard of care is met
  • Maintain all applicable certifications and licensure including CPR certification
  • Participate in regular staff meetings

COMPETENCIES

  • Experience with writing clinical assessments
  • Knowledge of electronic health record and practice management systems (e-Case Notes a plus)

  • Computer Skills – Proficient ability to use a computer and navigate electronic medical record. MS Office suite (Word, Excel, Outlook, PowerPoint) experience required

  • Flexibility – Ability to adapt easily to changing conditions and work responsibilities. Must also be able to work after school hours and some evenings and weekends
  • Organization Skills – Must have planning skills with demonstrated ability to work independently and exercise sound judgment
  • Efficient with strong attention to detail
  • Self-starter with strong customer support orientation (for internal/external customers) and demonstrated professional demeanor
  • Ability to pass all vital background checks/screenings

WORK ENVIRONMENT

  • In office position with community engagement; Hybrid opportunities available after 90 days.
  • 40-hour work week, Some weekend work will be required

Pay: $67,000.00 per year
