The Pediatric Psychiatry Collaborative (PPC) is an innovative state-funded grant program designed to ensure screening, assessment, diagnosis, and treatment of children presenting in pediatric primary care settings with behavioral health concerns. This is a key position in a state-wide initiative to expand mental health and care coordination in support of pediatric providers. This program is a collaboration to ensure pediatric screening and assessment, as well as provider education and guidance on behavioral health matters. It promotes integrated care management between pediatric providers, PPC clinical teams, child/adolescent psychiatrists, and community mental health agencies. This program provides a unique opportunity for clinicians to help pediatric healthcare providers to combine medical and psychiatric support. Our clinicians use clinical skills to intervene and help pediatric patients receive the support and care they need soon after symptoms appear rather than a reactive approach during a crisis.

  • Conduct telephonic biopsychosocial assessments of patients up to age 21, referred from pediatric medical providers.

  • Complete written clinical assessments in electronic medical record.

  • Formulate treatment recommendations for patient behavioral health needs.

  • Develop a plan of care, in collaboration with the patient/guardian, patient support persons and healthcare team that will provide maximum benefit for each patient as needed.

  • Assist with care coordination to treatment by identifying and providing local behavioral health resources to patients.

  • Facilitate internal referrals to AHS behavioral health services as needed.

  • Work closely with patient/guardians to provide psychoeducation and assistance with navigating resources.

  • Assist with identifying new community resources and maintaining resource list.

  • Work collaboratively with treatment team members, pediatric providers, patient guardians, schools, and other outside organizations as appropriate to ensure positive patient outcomes.

  • Conduct case reviews with Child/Adolescent Psychiatrist.

  • Work closely with Coordinator on other duties as assigned.

  • Participate in team meetings and other meetings as scheduled.

  • Complete all mandatory AHS trainings and competencies as assigned.

  • Master's degree in social work or related field

  • LCSW/LSW/LPC/LAC from the State of New Jersey
  • Previous experience working with pediatric/adolescent population preferred
  • Bilingual a plus

Competencies-

  • Demonstrates ability to conduct biopsychosocial assessments, screens effectively for suicide and violence risk as well as abuse and neglect.

  • Develops individualized treatment recommendations with measurable, patient centered goals, utilizes evidence based therapeutic approaches appropriate to the presenting problem(s).

  • Can build rapport and trust quickly with a range of patients, uses active listening skills, reflective statements and nonjudgemental language, conveys understanding and acceptance without reinforcing maladaptive behaviors.

  • Maintains clear, timely and accurate clinical notes that meet legal and departmental standards, manages caseload effectively.

  • Upholds professional ethical standards, maintains appropriate boundaries and therapeutic relationships.
