Home/Community-Based Recreational Therapist treating individuals with Traumatic Brain Injury, Spinal Cord Injury, Limb Loss and/or other Traumatic Injuries. Evaluate, facilitate and monitor services and client progress in relationship to established goals and objectives.

Status

Part-time (full-time status opportunity may be available for qualified candidate).

Responsibilities

Collaborate with client/family/team to complete age-appropriate comprehensive evaluation, including specialty evaluations and standardized tests/outcome measures as appropriate, establish plan of care including client centered SMART goals, and implement appropriate treatment interventions/strategies to maximize functional rehabilitation and recovery.

Adhere and maintain compliance with the policies/procedures of MI Rehab Solutions, HIPAA, appropriate professional practice act standards, including but not limited to scope of practice for licensure/certification, and applicable state/federal laws.

Complete timely, ongoing formal and informal reassessment, revising/updating plans of care as appropriate/required. Maintain and submit accurate and timely documentation based on MI Rehab Solutions policy and procedure guidelines, in accordance with local, state, federal and funding source regulations.

Provide ongoing delegation of duties and/or supervision, as appropriate/required, for student clinicians, new employees, or others per state guidelines and practice act.

Effectively communicate/collaborate with other internal and external team members in a timely and professional manner to best serve client's needs, including but not limited to:

  • Telephone and electronic communication methods
  • Client centered and professional team meetings
  • Attending physician/other provider appointments as appropriate
  • Staff meetings/trainings

Completion of education and training as required by regulatory bodies or supervisor

Travel as needed to provide home/community-based rehabilitation services for clients.

Support MI Rehab Solutions programming/operations as appropriate and complete related duties as assigned by supervisor.

Assist with identifying challenges and related solutions for best possible practice and implementation of optimal customer service.

Qualifications

  • Licensure/Certification:
    Bachelor's degree or higher, with concentration in therapeutic recreation, or equivalent. Certification by the National Council of Therapeutic Recreation
  • Experience:
    3-5 years in related field(s) experience preferred.
  • Other:
    excellent computer proficiency
