Located in central Florida, AdventHealth Well 65+ is part of a large, integrated faith-based health system providing extensive healthcare resources and community benefits.AdventHealth Well 65+ is seeking an experienced, Board Certified Family or Internal Medicine to join our team and provide personalized care to patients 65-years of age and older. This practice exclusively focuses on Medicare Advantage patients. At work, you will experience: More Time with Patients: Up to 40 minutes with patients to provide comprehensive, personalized care Smaller Patient Panel. Max patient panel of 700. Lower Daily Patient Census. 14-16 patients per day. Clinic Support Team. Includes RN Case Management, 1.5 Health Coaches (MA s) per provider, Social Worker, Pharmacy, Care Coordinators, Welcome Coordinator, HCC Coding Support, and Administrator.
