Job Title: Physical Therapist Profession: Physical Therapist Specialty: Rehab Duration: 13 weeks Shift: Days Hours per Shift: 8 hours M-F; Times are flexible as long as consistent and communicated with DOR Experience: 1 year of PT experience required License: Missouri State License required Certifications: None specified Must-Have: LTC or Rehab experience required, New Grads OKAY with Level II experience Description: Effectively screens, evaluates, and rehabilitates patients with physical functioning disorders and/or wound care. Develops appropriate plan of care following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practice to support clinical interventions. Administers various procedures as part of the rehabilitation plan including manual techniques, ambulation, therapeutic exercises, modalities, and use of supportive and assistive devices. Assesses patient progress and adjusts treatments accordingly in the clinical record per regulatory and clinical practice requirements. Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made based on outcomes. Provides consultation and counseling to patients, as well as families, caregivers, and other service providers related to physical disorders. Effectively communicates with supervisors and other health team members regarding patient progress, barriers, and treatment plans. Provides comprehensive discharge summaries of services provided per regulatory and clinical requirements. Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy. Accurately records treatment minutes for services provided per all regulatory and clinical requirements. Participates in required meetings including patient care conferences, utilization review meetings, and rehabilitation meetings. Participates in required trainings, in-services, and conferences. Able to obtain any necessary DME or supplies per regulatory and clinical practice requirements. Able to supervise and support physical therapist assistants, rehabilitation aides, and/or students. Adheres to professional practice standards and code of conduct during practice as a Physical Therapist.
