Essence Hospice Care of Arizona is seeking a skilled LPN to join our team in providing high-quality hospice care in Tucson, AZ. As an LPN, you will work under the supervision of the Registered Nurse Case Manager to provide direct nursing care and support to patients in their homes.**Responsibilities:**- Provide direct nursing care as indicated by the plan of care under the supervision of the Registered Nurse Case Manager.- Participate in the planning and coordination of patient care.- Administer prescribed medications according to law as permitted within the scope of individual license.- Provide patient/caregiver teaching as appropriate.- Assist with maintaining accurate and up-to-date patient records.- Collaborate with the interdisciplinary team to ensure the highest quality of care for patients.- Adhere to all policies, procedures, and regulations of Essence Hospice Care of Arizona.**Requirements:**- Current LPN license in the state of Arizona.- Minimum of 1 year of nursing experience, hospice experience preferred.- Strong communication and interpersonal skills.- Ability to work independently and as part of a team.- Compassionate and empathetic attitude towards patients and families.- Must pass a background check (Note: Essence Hospice Care of Arizona complies with regulations that prohibit inquiring about criminal backgrounds during the hiring process).If you are a dedicated LPN looking to make a positive impact in the lives of patients and their families, we encourage you to apply for this rewarding part-time position at Essence Hospice Care of Arizona.
