Family Practice/Primary Care Nurse Practitioner

Job Summary: Field-based, making Primary Care House Calls, conduct comprehensive assessments, develop care plans, and provide continuous care for older adults in their homes, collaborate with a multidisciplinary team to deliver integrated care. Utilize your expertise in geriatrics and primary care to make a meaningful difference in the lives of your patients, Fluent Spanish is ideal.
Job Responsibilities:
Field-based, making Primary Care House Calls.
Conduct comprehensive assessments, develop care plans, and provide continuous care for older adults in their homes.
Collaborate with a multidisciplinary team to deliver integrated care.
Utilize your expertise in geriatrics and primary care to make a meaningful difference in the lives of your patients.
Fluent Spanish is ideal.
Qualification/Experience:
Valid Georgia ARNP license.
Reliable transportation and current state driver's license and proof of auto insurance.
Current DEA license.
Medical Specialty:
Geriatrics
Primary care
Additional Information:
It involves providing primary care for older adults (65+) in their homes (house calls). This is not home health or home care, but rather primary care delivered at home.
