Full-Time Registered Nurse Hospice (Monmouth County)

At VNA Health Group, were transforming healthcare by empowering nurses to deliver compassionate care that helps patients live with dignity and comfort.

As a Full-Time Hospice RN serving Monmouth County, youll assess patient needs, manage symptoms, create individualized care plans, and support families through the end-of-life journey.

Responsibilities
  • Complete a two-week orientation focused on hospice care practices, EMR systems, and safety protocols.
  • Assess patients with advanced illnesses and create individualized hospice care plans.
  • Provide education, emotional support, and guidance to patients and families as they navigate the end-of-life journey.
  • Collaborate closely with VNA clinicians and external providers to ensure seamless, compassionate care.
  • Document patient status updates and adjust care plans as needs and goals evolve.
Qualifications
  • RN with Bachelors degree in Nursing
  • Certification in Hospice Palliative Care Nurse and Critical Care and Critical Care skills preferred
  • 1+ year of supervised clinical experience as a registered nurse
  • Excellent communication skills
  • Strong computer skills
  • Excellent organizational and interpersonal skills

Working Conditions/Physical Demand: Working indoors/outdoors with clinical equipment and use of phone and computer; car and valid NJ Drivers license

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $60,000 to USD $110,000. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
