Pay Range: $33.15 $59.67 per hour
Overview: As a Registered Nurse with CommonSpirit Health at Home, you will deliver expert clinical services at patients homes, coordinating with multidisciplinary teams and fostering meaningful relationships with each patient and family.
Responsibilities- Deliver compassionate, high?quality nursing care tailored to each patients needs.
- Develop and adjust individualized care plans to ensure optimal patient outcomes.
- Collaborate with physicians, therapists, and other healthcare professionals.
- Supervise your own caseload and mentor LPNs, LVNs, and Home Health Aides.
- Provide education and support to patients and families.
- Maintain thorough documentation to meet regulatory and operational standards.
- Utilize the latest technology and best practices to enhance patient care.

- Graduate of an accredited nursing program; active RN license in the state served.
- At least 1 year of nursing experience (Home Health preferred).
- Strong critical thinking, clinical expertise, case management, and passion for patient care.
- CPR/BLS certification.
- Valid drivers license.
