divh2Per Diem (PRN) Occupational Therapist at Cedar Manor Nursing and Rehab/h2pWe are looking for an OT to join our in-house team of outstanding professionals that service our residents through person-centered care to achieve positive outcomes/ph3Occupational Therapist Responsibilities:/h3ulliConsulting with patients to learn about their physical condition and impact on daily occupations, well-being, and quality of life./liliAssessing and interpreting patient evaluations and test results./liliDeveloping personalized treatment plans using a variety of treatment techniques to maximize safety and performance in activities of daily living (ADL) and instrumental activities of daily living (IADL)./liliAssess clients home and other environments and advise on alterations consistent with their needs and capabilities./liliProvide recommendations for home modifications and adaptive equipment with training as needed./liliEnsure all treatment delivered is in accordance with the established plan of care./liliMonitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration./liliProvide clinical support and education to patients, family members, and caregivers./liliCollaborate with other members of the interdisciplinary team to ensure quality patient care is provided./liliEnsure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements./liliComplying with all rules, regulations, and procedures./liliAttend required meetings as designated by the Director of Rehab./li/ulh3Occupational Therapist Skills:/h3ulliSNF/Long term care experience preferred./liliCurrent knowledge of treatment practices./liliKnowledge of Net Health Electronic Documentation System preferred./liliAbility to manage patients with different types of personalities./li/ulh3Occupational Therapist Requirements:/h3ulliActive/Valid Texas license in Occupational Therapy./liliIn good standing with all regulatory agencies and licensing boards./li/ulh3Career Advantages:/h3ulliIn-house Rehab (non-contracted)/liliNew Grads welcome to apply/liliCompetitive Compensation./liliFlexible Schedules/li/ul/div
