Job SummaryProvide intermittent nursing service to patients in their homes under a plan of care established in collaboration with the patient's physician and according to agency policy and procedures, using hospice concepts and promoting quality of life for the terminally ill.
Responsibilities- Admit and readmit patients, and provide intermittent nursing service to patients and their families in compliance with agency policies and procedures.
- Develop and implement an appropriate care plan and therapeutic goals with the patient/family.
- Evaluate patient safety on a 24-hour basis and exercise appropriate clinical judgment in coordination of patient care.
- Document all services provided in the clinical record in accordance with state and federal regulations and agency policy.
- Manage work assignment in an effective and efficient manner, and maintain productivity as established by the agency.
- Graduate of an NLN-approved school of nursing, associate or baccalaureate degree program.
- One year of recent medical/surgical nursing experience, with additional experience in community health nursing preferred.
- Possession of a current license to practice as a Registered Nurse in the State of Connecticut.
- Possess adequate transportation and show proof of satisfactory automobile insurance.
Work is performed in patients' homes and in an office environment, with some exposure to communicable diseases and bodily fluids.
