Integrated Case Manager

Sentara Health Plans is hiring a Care Coordinator in Culpepper, Page County and the surrounding areas This is a full-time, permanent position (40 hours) with standard working hours of 8am to 5pm EST, Monday to Friday. The position requires both in person/face-to-face assessments and remote telephonic assessments for the LTSS Waiver population in Culpepper, Page County, and the surrounding areas. Candidates must reside in Culpepper, Page County, and the surrounding areas.

Job responsibilities include performing a variety of casework duties and providing case management services to patients, families, and designated caregivers. Must develop, participate, and monitor multidisciplinary collaboration of services to patients where appropriate. Assist adult patients and their families with personal and environmental difficulties associated with medical condition up to and including at time of terminal illnesses.

Education: Must possess a degree in Health & Human Services or one of the following related fields:

Certification/Licensure: None required.

Experience: Long Term Care- 1 year; Health Plan- 1 year; Medicaid- 1 year REQUIRED. At least one year of mental health experience preferred.
