Occupational Therapist

Reliable Home Care Providers, Inc. is seeking a licensed Occupational Therapist passionate about serving individuals in their homes. This part-time position is ideal for a clinician seeking to gradually build their caseload with the support of an experienced and responsive administrative team. The role offers scheduling flexibility and the opportunity to work with a diverse patient population.
Primary Responsibilities- Provide direct, skilled occupational therapy services in patients' homes
- Conduct evaluations, develop individualized care plans, and establish patient-centered goals
- Deliver therapeutic interventions aimed at improving functional independence
- Collaborate with physicians, case managers, and family caregivers to ensure continuity of care
- Complete timely, compliant, and accurate clinical documentation in accordance with regulatory standards
Preferred Candidate
- Licensed Occupational Therapist in the State of Illinois (required)
- At least 1 year of home health or community-based therapy experience preferred but not required.
- Familiarity with electronic medical record (EMR) systems (WellSky Home Health)
- Strong clinical judgment, communication skills, and commitment to professional standards
- Compassionate, punctual, and able to work independently
- Must have a valid drivers license, reliable transportation, and proof of auto insurance.
