What You'll Do- Deliver skilled nursing care to patients in their homes, following physician orders and individualized care plans.
- Conduct initial assessments and collaborate with Population Health Case Managers.
- Supervise and support LPNs and Home Health Aides.
- Focus on achieving excellent patient outcomes and satisfaction.
- Graduate of an accredited nursing program.
- Active RN license in Delaware.
- Minimum 1 year of nursing experience (Home Care or Acute Care preferred).
- Valid driver's license and reliable transportation.
- Willingness to work 1 weekend/month and 2 holidays/year (1 summer, 1 winter).
