Nurse Practitioner / Physician Assistant - Cape Cod, MA
Type: Full-Time or Part-Time
Setting: Skilled Nursing Facilities & Assisted Living Facilities
Work Location: In-person
Responsibilities
- Provide primary care visits 3-4 days per week
- Perform medication reconciliations and annual wellness visits
- Examine patients and review medical records
- Order, review, and interpret diagnostic tests (labs, X-rays, etc.)
- Diagnose medical conditions and illnesses
- Develop treatment plans for chronic and acute conditions
- Prescribe appropriate medications
- Maintain accurate and timely records
Qualifications
- Valid Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of MA
- Graduation from an accredited NP or PA program
- New graduates are welcome-especially those with clinical rotations or experience in primary care, internal medicine, geriatrics, or skilled nursing settings
- Strong assessment, diagnostic, and clinical decision-making skills
- Ability to work independently in a round-and-go model while collaborating with a multidisciplinary team
- Excellent communication and patient-education skills
