We are seeking a Licensed Social Worker for a Temporary assignment - approximately 12-15 weeks.

  • Full-Time schedule.
Licensed Social Worker Qualifications:
  • Current, unencumbered OH Social Worker License required.
  • Bachelor’s Degree in Social Work required.
  • A minimum of one year of Social Work experience.
  • Experience as a Social Worker in long-term care preferred.
Licensed Social Worker Introduction:

Under the supervision of the Administrator, the Licensed Social Worker is responsible for the planning, organizing, implementing, evaluating and directing of the Social Services program in accordance with current, federal, state regulations, industry best practices. Leading Social Work and Case Management staff to assist the residents to attain or maintain the highest practicable physical, mental and psychosocial well-being, navigating the health care system. Responsible for coordination of discharge planning and case management.

Licensed Social Worker Job Summary:
  • Guide facility in resident advocacy, protection and promotion of residents’ rights.
  • Manage department staff to allocate resources, supervise, plan and implement individual resident plans of care.
  • Implement Social Work & Case Management initiatives and training of policies and procedures that govern social services.
  • Prepare for the long-term care survey. Interact with State surveyors.
  • Complete necessary forms and reports, including MDS and care plans.
  • Audit clinical records of residents on a regular basis.
  • Spearhead and coordinate interdisciplinary care plan meetings.
  • Maintain great relationships with community providers.
  • Support Advanced Directives process and work closely with the medical and nursing team to initiate the desired resident/health care proxy’s decisions.
