Job Description

POSITION TITLE: Team Manager - DSP

DIVISION: Community Services

SUPERVISOR: Director of Residential Services

STATUS: Non-Exempt

PURPOSE:

The purpose of this position is to assist in developing and implementing client-specific programs in settings as residential, day program, on the job or other community setting. The goal is helping individuals build strong social skills and encourage independence while creating an environment that allows the client to thrive. To ensure success in this position candidate should have respectable interpersonal skills and a passion for helping others. The company encourages staff to bring their own appropriate abilities and strengths to the job and encouraged to schedule time to share their interests, hobbies, or skills with the clients. The goal is to ensure the client feels supported to attain self-direction, inclusion, personal fulfillment, and productivity in all life areas.

ESSENTIAL JOB FUNCTIONS:
  1. Lead group class sessions and remain actively engaged in all lessons throughout the day.
  2. Plan and provide instruction in communication skills, daily living skills, socialization skills, fine and gross motor skills, perceptual skills, and any other area based on programming needs.
  3. Plan group activities based on schedule and persons served goals; prepare instructional materials for programming. Provide specific vocational and habilitation training to individuals based on assessment plan and needs.
  4. Maintain documents as required; prepares correspondence, write internal incident reports, and carries out similar administrative tasks associated with client care.
  5. Must be able and willing to use personal cell phone as needed, or company devise as available for time entry.
  6. Promote Health and Safety in all agency activities. Maintain a neat and orderly programming environment.
  7. Model company's expectations of behavior, culture, and mission as a representative of the agency.
  8. Respond appropriately to emergency health and safety situations, as well as behavior situations.
  9. Provide services in compliance with all funding sources and governing bodies (CARF, BQIS, State Board of Health, State and Federal Law). Maintain confidentiality adhering to HIPPA guidelines and company policies.
  10. Maintain professional communication with client, family members and other stakeholders which enable them to carry out habilitation training and help meet client's needs for personal care services. Maintain consistent, professional communication and feedback to supervisor.
  11. Must be capable of obtaining and maintaining certification in CPR, First Aid, CPI and Med Core A & B and perform if necessary. Any additional mandatory trainings as instructed.
  12. Comply with all agency policies and procedures including:
  13. Strict adherence to the company Medication Administration Policy for administering, storing, transporting, destroying, and reporting client medications.
  14. Strict adherence to State Protocol for reporting allegations of abuse, neglect and exploitation, and any illegal activities.
  15. This is an essential role that is necessary for the safety of our individuals and requires consistent attendance and punctuality.
  16. Use of medical adaptive equipment including, but not limited to: Wheelchair, Hoyer, Lift, Shower Chair, Blood Sugar Testing Equipment is required.
  17. Potential exposure to bodily fluids, pests, contagious pathogens, and/or bed bugs. Must be able to climb stairs on an occasional basis to access work areas, which are currently without elevator access.
  18. Advocate for client needs. Assist with the elimination of barriers, which restrict individuals with disabilities and continually work for their full inclusion in the community.
  19. Participate in periodic food or craft sales to supplement money to purchase supplies, as well as participate in other agency functions.
  20. Participate in meetings, committee assignments, and special projects as required.
REQUIREMENTS:
  1. B.S. degree in a human service field preferred. High School Diploma or equivalent required.
  2. At least one year of experience with the MR/DD population, preferably in a residential setting.
  3. Must be capable of obtaining and maintaining certification in CPR, First Aide, and NCI and perform if necessary, and Medication Administration.
  4. Must have excellent oral and written communication skills and maintain confidentiality with sensitive information.
  5. Maintain a valid Indiana driver's license, insurance on personal vehicle (minimum of PLPD with medical liability coverage), and maintain insurability as defined by the agency liability insurance carrier.
