We are searching for a dedicated and caring Hospice Licensed Practical Nurse (LPN) to join our compassionate team. Reporting to the Administrator or Executive Director, you will provide essential hands-on hospice care to patients and families in various settings including homes, assisted living, or facilities. Your commitment will make a significant impact during life's most challenging moments.
As a Hospice LPN, you will be responsible for:
- Providing direct nursing care to hospice patients, ensuring support for both patients and their families.
- Assisting the interdisciplinary team in assessing needs and executing personalized care plans.
- Promptly reporting any changes in patient conditions to the Registered Nurse and care team.
- Delivering empathetic care aligned with each unique plan of care.
- Acting as a trusted resource for families, offering support during difficult times.
Your key responsibilities will include:
- Delivering skilled bedside nursing under the supervision of a Registered Nurse, adhering to established protocols and the Nurse Practice Act.
- Monitoring patients and documenting any changes in condition, reporting as necessary.
- Administering medications and treatments per the care plan.
- Participating in infection control practices and following standard precautions.
- Supporting bereavement activities and family closure contacts as assigned.
- Participating in on-call duties and responding to patient needs during visits or emergencies.
- Assisting with death pronouncements, in accordance with state law and under RN supervision.
- Collaborating closely with hospice aides and other team members to ensure comprehensive care delivery.
About you:
- Graduate of an accredited Practical Nursing program.
- Current LPN/LVN license in the state of employment.
- Minimum of 2 years of LPN experience in hospice, oncology, long-term care, or home health; 3-5 years preferred.
- CPR certification required.
- Valid driver's license and current automobile insurance.
Essential Skills and Knowledge:
- Understanding of hospice care principles and the unique needs of terminally ill patients and their families.
- Exceptional patient assessment and clinical skills.
- Strong communication and documentation abilities.
- Familiarity with regulatory guidelines including Medicare, Medicaid, CMS, and JCAHO standards.
- Ability to work independently and collaboratively as part of an interdisciplinary team.

Compensation may vary within the provided salary range based on location, experience, education, skills, licensure, certifications, and departmental equity. The salary range for this position is $23.65 - $27.85.
