Dublin Post Acute Care is a skilled nursing and long-term care facility dedicated to exceptional resident care, cutting-edge therapy programs, and a supportive team environment. We are seeking a compassionate and motivated Physical Therapist (PT) to join our growing therapy team
What You'll Do:
- Provide high-quality physical therapy evaluations and treatments for residents requiring rehabilitation in a SNF/LTC setting
- Develop personalized treatment plans to increase mobility, function, and overall independence
- Collaborate closely with our interdisciplinary care team including OT, ST, Nursing, and Social Services
- Educate residents and families on care plans, safety, and progress
- Document patient progress in accordance with state and federal regulations
- Current and active Ohio Physical Therapist license required
- Prior SNF/LTC experience preferred, but not required
- Strong communication, clinical judgment, and teamwork skills
- Passionate about improving resident quality of life through dignified and results-driven care
- Supportive, collaborative therapy department
