Role Summary and Qualifications

The Acute Rehab RN Case Manager plays a vital role in delivering high-quality, patient-centered care by overseeing rehabilitative services for optimal patient outcomes. As a key member of the rehabilitation team, you will:

  • Provide orientation to patients and families about the rehab program, including case management, treatment plans, discharge criteria, and rights.
  • Serve as the coordinator for patient, family, and caregiver education, ensuring active participation in care discussions.
  • Collaborate closely with the Rehab Program Director and Facility Case Management Director in all departmental functions.
  • Document and monitor educational activities provided by the care team, promoting effective communication and goal setting during Family Conferences.

Qualifications:

  • Currently licensed as a Registered Nurse (RN).
  • Possess an Associate Degree or a Registered Nurse Diploma.
