As a Physical Therapist in Home Health, you'll complete patient assessments and evaluations to address their functional needs related to mobility, endurance, pain, and other medical conditions. You'll develop treatment plans to improve patient well-being and assess their progress towards recovery.

Key Responsibilities:

  • Evaluate patient's functional status and physical therapy needs, assisting in developing therapy plans.
  • Provide care within your scope of practice as defined by state laws, ensuring coordination with other healthcare team members.
  • Observe and report patient responses to treatment and any condition changes to nursing staff or the physician.
  • Administer treatments to relieve pain, restore function, and maintain maximum performance according to established standards.
  • Educate patients, families, and healthcare team members on the plan of care, functional limits, and progress toward goals.
  • Conduct patient visits according to the treatment plan, completing a progress note for each visit in line with LHC policy.
  • Complete evaluations and develop a physical therapy plan of care adhering to state-specific guidelines.
  • Document outcomes of evaluations, treatment goals and proposed frequency of care.

Required Qualifications:

  • Current Physical Therapy licensure in your practicing state.
  • Current CPR certification.
  • Valid driver's license and vehicle insurance, access to a reliable vehicle or public transportation.
  • Ability to deliver care respectfully, regardless of the patient's demographic characteristics.

Compensation is based on various factors including local labor market trends, education, work experience, and certifications.
