HOW DO YOU MAKE A DIFFERENCE?

In this part-time role, you provide comprehensive care to patients in the comfort of their own homes. Approaching every person with respect and compassion, you help patients address the social and emotional factors preventing them from achieving their goals. You join forces with nurses and other care professionals, coordinating your services to ensure patients have the full spectrum of their needs met. Communication is key as you educate patients and their caregivers in healthy ways to overcome their struggles and improve their well-being. Do you think you're the right fit for the job? WHAT DO YOU NEED?

  • Master's degree from a school of social work accredited by the Council on Social Work Education
  • Ability to work independently
  • Valid driver's license and reliable transportation
  • Excellent communication and problem-solving skills
