A few details about the role:

  • Collaborate with the interdisciplinary team to assess, plan, coordinate, and evaluate resident care.
  • Accurately document resident conditions and nursing needs, reporting observations and reactions in a timely manner.
  • Develop and monitor assignments for nursing assistants, adjusting based on resident care requirements.
  • Execute treatments as necessary, documenting reactions to medications and procedures.
  • Prepare, administer, and chart all medications efficiently.
  • Initiate and manage physician orders while ensuring clear communication.
  • Engage with families about changes in medications and resident health.
  • Establish protocols for ongoing review of care plans to reflect each resident's unique needs.
  • Oversee admissions, discharges, and transfers to ensure quality service delivery.

Here’s what you need to apply:

  • Graduation from an accredited nursing program as a Licensed Practical Nurse.
  • Active Licensed Practical Nurse license as required by state regulations.
