Position Overview:The Tennessee Department of Health (TDH) is seeking applications for the Medical Record Coordinator and Abstractor position, which is part of the Tennessee Birth Defects Surveillance System (TNBDSS) within the Family Health and Wellness Division. This full-time, hybrid position offers remote work with a requirement of 1-2 in-person days each month at the Central Office in Nashville, TN. Travel to Nashville is non-reimbursable. This role is renewable through July 2026.

As a subject matter expert in birth defects case management, the Medical Record Coordinator and Abstractor will be primarily responsible for the processing and abstraction of medical records related to birth defects cases.
Key Responsibilities:
  • Request and manage medical records for individuals with suspected or identified birth defects based on a developed algorithm.
  • Perform case verification for all reports received on key birth defect diagnoses.
  • Review assigned medical records (digital, electronic, and paper) to extract pertinent information and required data variables.
  • Cross-check information from multiple sources to ensure accuracy and completeness of abstracted data.
  • Enter abstracted data into the iCMS database.
  • Review data entered by non-clinical staff to ensure clinical accuracy.
  • Collaborate with the TNBDSS team to resolve issues and ensure that data processes are complete and accurate.
  • Maintain partnerships with facility Health Information Management (HIM) staff and offer feedback for process improvement.
  • Complete abstraction assignments on time and with minimal supervision.
  • Participate in regular meetings with the TNBDSS team.
  • Stay updated on patient care developments, data management best practices, birth defects, healthy pregnancy, and health equity.
Position Requirements:
  • An undergraduate degree (Associate's or Bachelor's) in a healthcare-related field is required; an RN is preferred.
  • A minimum of 1 year of experience in Health Information Management, medical coding, nursing, or medical record abstraction is required.
  • Extensive clinical experience (5+ years) in roles such as Medical Assistant, Phlebotomist, etc., may be considered in lieu of formal education (completion of a nursing program is preferred).
  • A clinical background in Pediatrics, Labor/Delivery, Congenital Birth Defects, or Maternal/Child Health is preferred.
  • Ability to maintain HIPAA compliance at all times.
  • Strong ability to work independently, with responsibility and efficiency.
  • Problem-solving skills and the ability to communicate effectively when assistance is needed.
