Position Details:
This is a full-time position with daytime shifts, conducting patient home visits in the Kissimmee, Celebration, St. Cloud, and Osceola County area. Specific zip codes include: 32836, 32821, 32830, 32837, 32824, 32827, 32832, 34773, 34771, 34772, 34769, 34744, 34743, 34741, 34746, 34758, 33896, 34759, 34747.
Your Essential Role:
As a Home Health Physical Therapist Care Manager, you will:
- Coordinate and oversee the care of a dedicated caseload of home health patients.
- Perform comprehensive assessments and develop individualized care plans in collaboration with patients, families, and physicians.
- Evaluate and document patient progress, adjusting treatment plans based on their needs and responses.
- Engage in continuous education of patients, families, and caregivers to ensure safe and effective care.
- Identify and implement performance improvement initiatives in line with best care practices.
Qualifications:
Minimum Qualifications:
- Valid Driver's License
- Physical Therapist License
- Current American Heart Association BLS Certification
- At least one year of relevant clinical physical therapy experience
- Graduation from an accredited physical therapy program recognized by the state
Preferred Qualifications:
- Experience in a Medicare-certified home health agency
- Master's or Doctorate in Physical Therapy
- Home Health Care Manager Certification or COS-C
- Knowledge of community resources for patient referrals
- Understanding of Home Health Regulations and Reimbursement
