Licensed Clinical Social Worker (LCSW) - Outpatient Dialysis Clinic
Location: Springfield, MA 01104
Schedule: Full-time, day shifts
Innovative Renal Care is hiring a Licensed Clinical Social Worker (LCSW) to support patients living with chronic kidney disease at our 21-station outpatient dialysis clinic in Springfield. You'll work with a collaborative care team serving in-center hemodialysis (ICHD) patients - helping them manage the emotional and social challenges of dialysis treatment.
Key Responsibilities
- Complete comprehensive psychosocial assessments at admission and annually
- Provide adjustment counseling, education, and emotional support to patients and families
- Coordinate access to transportation, insurance, financial aid, and community resources
- Actively participate in interdisciplinary care planning and care conferences
- Maintain clinical documentation and progress notes in compliance with ESRD guidelines
Qualifications
- Master's degree in Social Work (MSW) from an accredited program
- Active LCSW in Massachusetts (or eligibility for licensure)
- At least 2 years of clinical social work experience (dialysis or ESRD experience preferred)
- Strong communication and care coordination skills
- Committed to patient dignity, ethics, and collaborative care

Salary Range: $33.61 - $41.95 / hour depending on experience and qualifications
