Position Overview:
The Community Liaison plays a vital role in bridging our healthcare services with the community. This position focuses on business development while managing both inbound and outbound communications. As a local market leader, you will engage in B2B, B2C, and DTC relationships, representing our organization to referral partners, patients, and caregivers.
Key Responsibilities:
- Business Development & Outreach:
- Identify opportunities for educational in-services in the healthcare community and coordinate their implementation.
- Build and nurture strong relationships with referral sources including hospitals, physician offices, senior living communities, and other healthcare providers.
- Seek out new business opportunities to enhance patient census across service lines.
- Represent the organization at community events, health fairs, and professional networking functions.
- Patient & Family Engagement:
- Educate patients, families, caregivers, and legal representatives about the benefits and services of Hospice.
- Serve as a compassionate resource to assist families throughout the enrollment process.
- Communication & Coordination:
- Develop relationships with medical staff offices, hospitals, and nursing homes while advising on hospice and end-of-life information.
- Facilitate effective communication between referral sources and internal clinical teams.
- Ensure prompt follow-up on referrals and maintain accurate records of outreach activities.
- Market Leadership:
- Provide educational opportunities for physicians, physician office staff, hospital social workers, discharge planners, and nursing home staff.
- Analyze referral patterns from various sources to inform outreach strategies.
- Stay informed about the community’s healthcare market, competition, consumer demands, and policies that impact outreach and access for patients and families.
- Serve as the face of the organization in the local market, enhancing brand awareness and trust.
- Monitor market trends and competitor activities to guide strategic planning.
Qualifications:
- Experience in healthcare marketing, sales, or community outreach, preferably in home health, hospice, or a related field.
- Proven ability to achieve performance metrics such as referral volumes, patient admissions, community engagement, and timely follow-ups.
- Strong interpersonal and communication skills to engage diverse audiences.
- Familiarity with Medicare, Medicaid, and other payer sources is a plus.
- Understanding of hospice eligibility and care levels is preferred.
- Proficient with customer referral/source management systems to track outreach activities.
- Able to work independently and collaboratively in a fast-paced environment.
- Valid driver’s license and reliable transportation required.
Ideal candidates will have a history of using data to drive decision-making, improve outreach strategies, and contribute to overall business development goals.
